Removing the Power of Attorney from your Petal account

You may withdraw the Power of Attorney at any time, but you will need to let us know in writing by sending us a notarized copy of the request to remove the Power of Attorney to the address below:

Once we receive this paperwork, we’ll remove the Power of Attorney from the account.

 

Assigning a Power of Attorney to your Petal account

To assign Power of Attorney, you’ll first need to mail us a copy of the notarized Power of Attorney form. Please also include an additional note stating the member’s name, date of birth, and last 4 digits of the member’s card number. Our team will review the information you send us and determine whether or not the information is sufficient to confirm Power of Attorney for the account.

Once we receive those documents, we’ll send a confirmation letter to both the member and the newly designated Power of Attorney confirming the status change.
