To assign Power of Attorney, you’ll first need to mail us a copy of the notarized Power of Attorney form. Please also include an additional note stating the member’s name, date of birth, and last 4 digits of the member’s card number. Our team will review the information you send us and determine whether or not the information is sufficient to confirm Power of Attorney for the account.
